Please fill out and bring the following forms to your appointment:
Scheduling Sessions
I currently see clients Monday through Thursday from 8a – 7p. We can schedule standing appointments or as you need them. Meeting regularly will allow you to reap the greatest benefit from therapy. I emphasize the importance of attending all scheduled sessions.
Rescheduling
Life happens. A need to reschedule an appointment occurs. You will find me flexible in these situations and willing to find a new time works for both of us. Responsive and timely communication is immensely helpful when the need to reschedule occurs.
Fees
I am happy to discuss my fees with you during our initial in-person or phone consultation.
Payment
Unless there is a prior arrangement, full payment is required at the beginning of each session. Dependent upon your insurance plan you may be eligible for reimbursement. If this is the case, I will provide you the necessary forms to file for reimbursement.
Cancellations
There is no charge for appointments that are canceled at least 72 hours in advance. Except for unforeseen true/extreme emergencies, your regular fee will be charged for missed or canceled appointments with less than 72 hours notice.
Disclosure Statement
Washington State law requires that all marriage and family therapists present to new clients a disclosure statement that specifies the therapist’s background, experience, theoretical orientation, and approach to therapeutic services. The “Intake and Disclosure form” linked to above is intended to help you become an informed consumer about these aspects of my clinical practice and your rights as a client. Before our first session please download, review and sign this document.
